Chapter Five: Instructor Notification


Accommodation Memorandum
Each semester Instructor Notifications can be printed. This notification will list the accommodations you are requesting. You should take these notifications to your instructors and discuss how you would like to use the approved accommodations in his/her class.

You will need to pick the Instructor Notifications up in the Counseling Center before you attend your first class.

These notifications are usually ready the week before classes start, provided you have met with a Counselor at least two weeks before that date to register for classes. You must be registered in order to have the notifications printed for you.

Can the Notifications be given to only some instructors?
Yes. You should let your Counselor know well in advance which instructors you want to notify. DSS will then print only those notifications you desire. If you say that you want all of your instructors notified, DSS will print a notification for each instructor.

Will Counseling Center automatically print Instructor Notifications for me every semester?
No. Every semester you will need to make an appointment with your Counselor/Advisor and sign a form giving permission to produce Instructor Notifications. It will not be done automatically.

Procedure for delivering Instructor Notifications

  1. If you have registered for classes, you may pick up copies of your Instructor Notifications in the Counseling Center the week before classes begin. If you registered late, your notifications will be delayed one week. You will be asked to sign a release form when you pick up your forms. Notifications will not be sent directly to your instructors. It is your decision whether to give teachers your letters.
  2. Make arrangements to deliver your Instructor Notifications to your instructors. It is recommended that you schedule an appointment with each instructor to discuss your accommodation needs in a confidential setting. You may either:
    1. Phone or stop by your instructor’s office to make an appointment
    2. Ask the instructor before or after class what would be a convenient time to meet.

At the appointment, explain your accommodation needs clearly. Ask the instructor how they wish to handle specific procedures such as out of class testing, locating note takers, or arranging for printed materials to be enlarged. If you will be using a support person such as an attendant, scribe or interpreter, explain their role to the instructor. Let the teacher know what equipment you may be using in class. Learn how to describe your best learning style.

IF YOU DO NOT DELIVER YOUR INSTRUCTOR NOTIFICATION LETTERS TO YOUR INSTRUCTORS,
YOU WILL NOT RECEIVE ACCOMMODATIONS.
